An enhanced contact form with single-select, multi-select, short answer, and long answer field types. Useful for wholesale inquiries, service intake, RSVPs, and anything beyond a basic contact form.
🧰 How to install the block in your theme
- Open Theme Updater & Backups in your Apps admin
- Open Theme sections

Click View Details below the block. You can also click Preview to see the section in a demo storefront.Â

- On the right side of the page, confirm that you are seeing the "Ready to use in your theme" message, or click on the Subscribe & unlock all sections button to unlock the block.Â
- Select the theme, and click Customize theme to start adding the block.
- Hover over the existing blocks in the template to Add block and click on the Apps tab. From the options, select the block:Â
Add the section to a template
Click Edit theme to open the theme editor for the selected theme.
Open the template (e.g. home page, default product) and click (+) Add section in the sidebar.
Scroll down to the bottom of your default sections list and select 🧰 Contact form - Advanced.Â
Customize section settings
Click the 🧰 Contact form - Advanced section to customize general settings for the section.
Content
When Include phone number feature is enabled, a Phone number field will also appear below the Name and Email fields.
Section heading allows you to add a title for the section. The heading size can also be adjusted through the Heading size and Mobile heading size sliders.Â
The Text field can be used to display a short message above the contact form fields, and its font size can be adjusted using the Text size and Mobile text size range sliders.Â
Success message will appear after the customers submitted their inquiry.Â
LayoutÂ
Available Layout options can adjust the width of the contact form:
- Regular
- Wide
- Full-width
Use Content alignment to display the contact form in either a left-aligned or centered layout.
Button
While "Send" is the default button text for the contact form, you can also customize this label, and the button styling.Â
Choose colors for the button using the Button color selector.Â
Select colors
Choose colors for the section's Text color and Background color.
Adjust spacing
Use the Top padding and Bottom padding sliders to increase or decrease the padding of the section.
Use the Top margin and Bottom margin sliders to increase or decrease the margins of the section.
Customize blocks
Add blocks under the section by clicking (+) Add block
Text field
This block adds an input field that can be set as either a single-line or multi-line field for longer responses.
You can make this a required field by enabling the Required setting, which adds an asterisk (*) to the input field.
Dropdown
This block adds a dropdown field to the contact form, allowing users to select from multiple options. Like the Text field, it can also be set as required using the Required feature toggle.Â
Text field
Adds a text area that can be used for headings, descriptions, or additional messaging within the contact form.
Single-select
Adds a list of options where customers can select only one choice. This field can be set to Required.Â
Multi-select
Adds a list of options that allows customers to select multiple choices.
Date field
Adds a selectable date input for customers to enter a preferred or relevant date. This field can be set to Required as well.Â
